J1 waiver application process question

Those of you that have gone through the process did the application for J1 waiver require you to have a state medical license?

I have read online that the waiver application (ie application for Hib) requires one to have a medical license.

I have an expired J1 visa and have not applied for a license before it expired. To apply for a medical license I need a visa.

Can someone please clarify as it seems to be a circular problem.
